Move Well supports people with health conditions in increasing their physical activity levels in a way that’s right for them, using a personalised approach, focusing on what matters to each individual.
The scheme offers access to a wide range of physical activity opportunities tailored to specific needs and preferences. Activities and programmes are provided by various providers across different locations and formats.
Move Well includes a triage process, to help people find a way to be active that’s right for them. This process takes into account their health condition, risk levels, motivation, and preferences, and provides personalised recommendations.
A recent consensus statement highlights that the benefits of physical activity for people with long-term conditions "outweigh the risks." However, despite these benefits, various barriers can still make it challenging for individuals with long-term health conditions to remain active.
Move Well is designed to offer healthcare professionals a one-stop resource for physical activity support. It helps patients overcome barriers and empowers individuals with health conditions to take control of their physical activity choices.
Previously known as the Sheffield Physical Activity Referral Scheme (SPARS), this scheme has a longstanding history in the city. Move Well builds on the pioneering work of those who originally established the scheme, while adding additional support to enhance and develop the offer for local communities. Through co-production and collaboration, the re-launched scheme ensures even greater accessibility and personalised options.

Our scheme is designed to provide personalised support through two main referral pathways:
Healthcare Professional Referral
Self Referral
Both pathways lead to The Centralised Hub, where people will be offer a "What matters to you" conversation. This conversation helps determine the best way to support people.
From the Centralised Hub, there are two triage options:
Option 1: Navigator Triage
Option 2: Digital Triage (coming soon)
Based on the triage, people will be guided through the following options:
Category 1: For those with health conditions who would benefit from supervised exercise sessions with level 3/4 instructors.
Category 2: For those with health conditions who prefer less structured, community-based support options.
Category 3: For those with low-risk conditions or no health conditions.
Category 4: For those who need support with motivation or overcoming barriers.
If a specific activity or organisation has already been agreed with the patient, there is also an option for direct referrals into programmes to facilitate prompt access to the appropriate support.

Request a Call from a Move Well Navigator
Our navigators can assist patients in finding the physical activity option that’s right for them over the phone. To request a call from a Move Well Navigator, please complete a referral form. We aim to make contact within 5 working days.
The Navigator will go through some questions with the patient to understand their needs and provide options that best suit them. If they’d like to access physical activity programmes, they will be supported in the sign-up process, making onward referrals, and accessing the service.
Create an Account on Joy
On Joy, you have the option to create an account for your organisation or GP practice, which makes referrals easier.
With your account, you’ll benefit from:
SPINE integration: Patient details will auto-populate on the referral form, saving you time.
Personal inbox: Keep track of all your referrals, check their status, and easily communicate with navigators for updates.

Refer Directly to a Physical Activity Provider
If you’ve discussed physical activity options with the patient and know which programme they would like to join, please indicate this on the referral form under the "Please share some brief details regarding this referral" question. • To Contact Participants: To arrange appointments, provide updates, reminders, and information related to participation in the scheme.
• To Monitor and Improve the Scheme: To assess the effectiveness of the scheme and make necessary improvements.
• For Administrative Purposes: To manage and facilitate participation in the scheme.Data Storage and Third-Party Providers
Personal information is stored in a secure database provided by a Third-Party Provider. This provider is responsible for the secure storage and maintenance of data. We have contracts and agreements in place with the third-party provider to ensure that data is handled in compliance with applicable data protection laws and regulations.Data Sharing and Disclosure
The aim of the scheme is to connect people to organisations providing physical activity support. We will only share personal information with third parties in the following circumstances:
• With Consent: When people have explicitly given us permission to share their information.
• For Legal Requirements: When required to do so by law or to protect the rights, property, or safety of our organisation or others.When people sign up, we'll ask for some basic details like their name, contact info, and request that they copy information from this chat. These details will be processed by the Move Well team at Sheffield City Council and then passed on to the organisation running the physical activity programme they’re interested in. They’ll use the information to arrange an appointment and offer support.
• Details will be securely stored on a third-party platform called Joy. By joining the programme, participants will be agreeing to the terms set by the activity provider, who will be responsible for delivering the services and keeping their information safe.Data Security
We implement appropriate technical and organisational measures to protect personal information against unauthorised access, alteration, disclosure, or destruction. However, no method of transmission over the internet or electronic storage is completely secure.People's Rights
People have the following rights regarding their personal information:
• Access and Correction: They can request access to or correction of their personal information.
• Withdrawal of Consent: They can withdraw consent to participate in the scheme at any time, which will result in the cessation of further contact from us.Additional Information
