Many people with health conditions find that being active helps them manage their condition and symptoms, as well as improving their general well-being. We understand that getting started can be challenging, and figuring out the right activity for you isn’t always easy.
Move Well is here to help you find the right support and keep you motivated.
Christmas Closing - Please note the scheme will be closed from Friday 20th December to Monday 6th January for the holidays. If you have been referred to the service we will contact you as soon as possible in the new year.

Request a Call from a Move Well Navigator
Our navigators can assist you in finding the physical activity option that’s right for you over the phone. To request a call from a Move Well Navigator, please complete a referral form.
The Navigator will go through some questions with you to understand your needs and provide options that best suit you. If you’d like to access physical activity programmes, we will explain the sign-up process, make onward referrals, and support you in accessing the service.
DIGITAL TOOL (COMING SOON)
We’ve created a user-friendly digital tool to help you find physical activity support tailored to your personal circumstances.
By using this tool, you'll receive personalised recommendations for the physical activity options available in Sheffield. It will provide details on activities that could benefit your health condition, including locations and costs. You can also ask questions about these activities.

To Contact You: To arrange appointments, provide updates, reminders, and information related to your participation in the scheme.
To Monitor and Improve the Scheme: To assess the effectiveness of the scheme and make necessary improvements.
For Administrative Purposes: To manage and facilitate your participation in the scheme.
Data Storage and Third-Party Providers
Your personal information is stored in a secure database provided by a Third-Party Provider. This provider is responsible for the secure storage and maintenance of your data. We have contracts and agreements in place with the third-party provider to ensure that your data is handled in compliance with applicable data protection laws and regulations.
Data Sharing and Disclosure
The aim of the scheme is to connect people to organisations providing physical activity support. We will only share your personal information with third parties in the following circumstances:
With Your Consent: When you have explicitly given us permission to share your information.
For Legal Requirements: When required to do so by law or to protect the rights, property, or safety of our organisation or others.
When you sign up, we'll ask for some basic details like your name, contact info, and request that you copy information from this chat. These details will be processed by the Move Well team at Sheffield City Council and then passed on to the organisation running the physical activity programme you’re interested in. They’ll use your information to arrange an appointment and offer you support.
· Your details will be securely stored on a third-party platform called Joy. By joining the programme, you'll be agreeing to the terms set by the activity provider, who will be responsible for delivering the services and keeping your information safe.
Data Security
We implement appropriate technical and organisational measures to protect your personal information against unauthorised access, alteration, disclosure, or destruction. However, no method of transmission over the internet or electronic storage is completely secure.
Your Rights
You have the following rights regarding your personal information:
Access and Correction: You can request access to or correction of your personal information.
Withdrawal of Consent: You can withdraw your consent to participate in the scheme at any time, which will result in the cessation of further contact from us.
